Office Hours with Olivia Hayden, Community Resource Navigator for Wellfleet and Eastham

The Outer Cape Health Services Community Navigator program helps assist residents with unmet medical, behavioral health, or human service needs through collaboration with local agencies, providers, and community groups.

Services include:

• Referring and connecting patrons to community resources such as social services, mental health services, substance abuse services, medical care, homelessness/housing assistance, and case management

• Assisting in navigating the variety of services available

• Problem-solving barriers to access

• Coordinating care
